Hospice eligibility is determined by a licensed hospice agency or physician and typically requires a terminal prognosis of six months or less if the illness runs its natural course. Diagnoses can include cancer, advanced dementia, heart failure, COPD, and many others. Our team can help you connect with a hospice agency to determine eligibility.

Absolutely — family involvement is central to our approach. We actively include families in care planning, provide regular updates, and ensure your loved one’s wishes and your family’s values are honored at every step. You are never an outsider in your loved one’s care here.

We work collaboratively with most licensed hospice agencies in the Sacramento area. If your family has a preferred agency, we will coordinate with them directly. If you need guidance selecting an agency, our admissions team is happy to share recommendations and facilitate the introduction.

Medicare Part A typically covers hospice agency services — including medical visits, medications related to the terminal diagnosis, and chaplaincy. The residential care portion at Modern Care is generally covered through private pay, long-term care insurance, or other benefit programs. Our team will walk you through all available financial options during your first conversation with us.
